Expert uncovers hidden risks linking pre-stroke depression to poor recovery outcomes‎

Depression doesn’t just affect the mind; it can shape how the body recovers from life-threatening illnesses.

In a powerful study published in the Journal of Vascular Nursing, Dr. Marvin Okon, a physician and public health researcher at Clemson University, reveals how depression before a stroke can influence a patient’s ability to walk again after treatment.

The research, conducted in collaboration with the University of South Carolina School of Medicine, analyzed more than 500 patients with strokes treated with recombinant tissue plasminogen activator (rt-PA), a medication used to dissolve blood clots in the brain.

The findings suggest that patients who experienced depression before their stroke faced significantly lower chances of regaining mobility, compared to those without pre-stroke depression.

‎‎“Pre-stroke depression may silently set the stage for poorer recovery,” Dr. Okon explained, noting that emotional health can alter brain and motor functions long before a stroke occurs. 

‎‎His work challenges the traditional focus on post-stroke depression by highlighting the importance of mental health even before a cerebrovascular event.

‎‎The study found that patients with pre-stroke depression had multiple clinical risk factors that reduced their odds of walking independently after therapy. These included congestive heart failure, previous transient ischemic attacks (TIAs), higher stroke severity scores, and use of antihypertensive medication. 

‎‎By contrast, those without pre-stroke depression who suffered from carotid artery stenosis or peripheral vascular disease were more likely to show improved ambulatory outcomes.

‎‎According to the report, over 60 percent of stroke survivors experience reduced mobility, and depression can worsen that outcome. Dr. Okon’s team suggests that depression may alter the brain’s motor pathways, hindering recovery even after successful treatment. “Our findings indicate that emotional and vascular health are closely intertwined in determining recovery,” the authors wrote.

‎‎This discovery opens new possibilities for early intervention. By screening stroke patients for depression history before treatment, physicians could better predict rehabilitation needs and tailor therapy to improve outcomes. 

It also calls for stronger integration between neurology and mental health services within hospitals.

‎‎Dr. Okon’s expertise in both clinical medicine and public health research uniquely positions him to address such multidisciplinary challenges. Trained as a medical doctor at Obafemi Awolowo University in Nigeria, and later completing advanced health research studies at Clemson University, South Carolina, United States, he bridges frontline clinical experience with cutting-edge public health research.

‎‎His professional journey spans from providing patient care in Nigeria’s hospitals, such as Adeoyo Specialist Hospital, Delight Hospital Ibadan, and The University College Hospital Ibadan, to leading high-level research in the United States with specialized research teams at prestigious institutions like Clemson University, PRISMA Health South Carolina, and the American Cancer Society.

Dr. Okon utilizes national health databases, such as NHANES and BRFSS, to examine trends in disease outcomes, disparities, and preventive care. His current research also focuses on the use of data from the North American Association of Central Cancer Registries (NAACCR) to explore the impact of National Health policies, such as the expansion of Medicaid from the Affordable Care Act, on cancer outcomes ranging from early diagnosis to mortality and attendant place-based disparities. 

‎‎This study on stroke and depression builds on his growing body of work examining the complex relationship between chronic diseases and social or psychological factors. 

‎‎Previous publications by Dr. Okon have explored lung cancer screening disparities, pre-stroke conditions, and health outcomes among underserved populations, demonstrating his commitment to addressing inequities in healthcare.

‎‎Colleagues praised Dr. Okon for blending empathy and analytics—a rare combination in academic medicine. 

“He understands the human story behind every dataset,” one collaborator remarked. “His work reminds us that recovery is more than medicine; it’s the intersection of mind, body, and community.”

‎‎Beyond research, Dr. Okon contributes to medical education by teaching epidemiology and medical terminology to pre-med students at Clemson University. 

His mentorship emphasizes critical thinking, compassion, and scientific integrity, qualities evident in his published work.

‎‎By linking clinical data to real-world health systems, he aims to influence how future healthcare frameworks address both prevention and recovery.
